Registration Term and Conditions applied to all our organizations as listed below –

Details

*The fees taken for ARDSI Nashik support group as Life membership and/or Sunday special clinic are non-returnable.

* It is mandatory to fill all details in the forms issued online or offline.

*The donations collected for DRMK MEMORY FOUNDATION and ARDSI have IT exemption under 80C and are non-returnable.

*ARDSI, DRMK MEMORY FOUNDATION volunteers should register with life membership of both the organizations separately
 
Volunteers must fulfil the enrollment criteria as follows-

  1. The person willing to be a volunteer should be 18+ years
  2. The person must not have any contagious disease
  3. The person must not have any kind of criminal charges
  4. More than one family member of any family shall not be employed if otherwise any need arises, and the committee holds all the rights for such permissions
  5. In case of any kind of misbehavior/misconduct seen by any volunteer, the committee holds the rights to immediately withdraw life membership filing legal action against such behaviours.
  6. The person must not have any kind of addiction
  7. We strictly follow non-smoking/Non-alcohol/Non-drugs Policy, anyone violating our policies shall be terminated from concerned work immediately, without any grace period.
  8. If a person found to have abused –(Physical/mental/sexual/financial/legal), any patient/colleague/caregiver/staff member/anyone connected with the DRMK MEMORY FOUNDATION, ARDSI Nasik Support group) shall be terminated from the concerned position and legal action shall be taken immediately
  9. Important Note: All the organizations listed above will prioritize persons directly or indirectly NOT connected to any Govt. employees, persons covered under Govt. schemes/minority facilities/family or individual annual income above 3 lacs for small-scale industry employment or skill-based training provided under our organizations.
  10. However, becoming volunteer of any of the above said organizations have no restrictions.
  11. The terms and conditions are final authority of the directors and managing committee.
  12. The above conditions shall follow the existing National & UN convention legal framework as applicable.

DONARS

*For donars the same terms and conditions apply

*Acceptance of donations for both organizations "ARDSI - NASHIK SUPPORT GROUP" & DRMK MEMORY FOUNDATION shall subject with due consent of all directors of the DRMK MEMORY FOUNDATION.
 
Term and conditions for patients and caregivers

  • In case of any kind of misbehavior/misconduct seen by any caregiver or patient-otherwise not related to clinical condition, the committee holds the rights to immediately withdraw clinical support filing legal action against such behaviours.
  • A caregiver or patient – otherwise mentally sound, if found to have abused –(Physical/mental/sexual/financial/legal), own or other
  • patient/colleague/caregiver/staff member/anyone connected with the Mental Health Aims, DRMK MEMORY FOUNDATION, ARDSI Nasik Support group, shall not be entertained and legal action shall be taken immediately
 
*The above conditions shall follow the existing National & UN convention legal framework as applicable.
 
Terms and Conditions for registration for UMACE screening test training and “BrainNext” rehab tools training, as follows

* Those who wish to enrol for “UMACE” and “BrainNext” training are requested to note
  that some of the Seminars and Workshops listed as follows are mandatory to understand
  the subject and it’s application in detail.

- SEMINAR SERIES NO. 1 – CAREERS IN MENTAL HEALTH
  (MANDATORY SEMINARS - No. 3, 5, 6, 8, 9, 10)

- ADVANCED SEMINAR SERIES ABOUT NEUROPSYCHOLOGY & MEMORY & COGNITIVE
  REHABILITATION - ALL LISTED 5 SEMINARS

* After completing the seminars the registration shall be processed for the WORKSHOPS to
  be attended that is MANDATORY
- IF Training for UMACE – WORKSHOP NO. 1 Understanding “UMACE” screening and
  Application
- IF Training for UMACE-VIP – WORKSHOP NO. 1 & 2 both

*IF you wish to enrol for “BrainNext” training PART 1

- After completing the above listed SEMINARS, you are requested to attend

WORKSHOP NO. 3 – “BrainNext” for Part – 1

*IF you wish to enrol for “BrainNext” training PART 2

- After completing the above listed SEMINARS, you are requested to attend
 WORKSHOP NO. 4 – “BrainNext” for Part – 2
